Lift Exercise Rehabilitation is committed to protecting your personal and health information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles. This policy outlines how we collect, use, store, and disclose your information when providing Exercise Physiology services.

Collection

We collect personal and health information that is necessary to provide you with safe and effective clinical care, including your contact details, health and medical history, and information relevant to your assessment, treatment, and funding pathway. We collect this information directly from you through intake forms, clinical assessments, and consultations. We may also receive information from your referring practitioner, GP, specialist, insurer, or case manager with your consent or as required under your funding scheme.

Use and Disclosure

Your information is used to provide clinical exercise physiology services, develop and manage your exercise program, communicate with your referring practitioner and care team, and meet reporting and compliance requirements under relevant funding schemes including Medicare, DVA, WorkCover, NDIS, Comcare, motor vehicle insurance, and Defence. We will not disclose your information to anyone outside of your care team or funding pathway without your consent, unless required by law.

Use of AI

Lift Exercise Rehabilitation uses AI-assisted tools within PracSuite to support efficient and accurate clinical documentation. These tools operate entirely within the PracSuite system and are used solely to assist with session notes and data management. No independent clinical decisions are made by AI. All information processed by AI tools remains securely stored within PracSuite and is not shared with external parties.

Access to Your Records

You have the right to request access to your personal and health information held by Lift Exercise Rehabilitation, and to request correction of any information you believe is inaccurate, out of date, incomplete, or misleading. To make a request, please contact us in writing at admin@liftexerciserehab.com.au. We will respond within 30 days. In some limited circumstances, access may be refused in accordance with the Australian Privacy Principles — if so, we will provide you with written reasons.

Storage and Security

Your information is stored securely in PracSuite, our practice management system. We use encryption, password-protected devices, and regular backups to protect your data. Paper records, where they exist, are stored securely and accessible only to authorised personnel. Health records are retained for a minimum of seven years from the last date of service, or until the client turns 25 years of age if they were a child at the time of treatment, in accordance with Western Australian requirements.

Data Breach

In the event of a data breach that is likely to result in serious harm, Lift Exercise Rehabilitation will notify affected individuals and the Office of the Australian Information Commissioner (OAIC) in accordance with the Notifiable Data Breaches scheme under the Privacy Act.

Complaints

If you believe your privacy has been breached, you are encouraged to contact Lift Exercise Rehabilitation at admin@liftexerciserehab.com.au so we can address your concerns. If you are not satisfied with our response, you may lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at www.oaic.gov.au.
